How much do services cost?
We accept all 5 Louisiana Medicaid Insurances and have private pay options as well (see below).
How do I get started?
Call our office to schedule a assessment (typically 1-1.5 hours) at 225-364-2550. Once your assessment is processed a Service Coordinator will contact you to set up your first session.
Assessments are typically completed Monday - Friday between 8:30 am - 5 pm, however alternate arrangement may be made in certain circumstances.
Sessions are scheduled during the day and evening including weekend hours.
Doctor appointments are scheduled by our main office by appointment only during regular business hours.
What ages do you serve?
We serve clients ages 4 to 99.
What type of insurance do you accept?
LSF accepts all Louisiana Medicaid insurances including: Healthy Blue 844-521-6941, AmeriHealth Caritas 888-922-0007, Aetna 855-242-0802, Optium United Healthcare 866-675-1607, Louisana Healthcare Connections 866-595-8133
What if I don’t have insurance?
LSF offers self-pay services at our office. All services are to be paid for before the service is provided by check or money order. Self pay services require a minimum of a three month commitment. Below is a full list of self-pay services.
Self-pay: Assessment - call for fees
Provided by a licensed staff at the office for 50 minutes:
Individual Outpatient Therapy (0-20) - call for fees
Individual Outpatient Therapy (age 21+) - call for fees
Family therapy - call for fees
Couple therapy - call for fees
Group therapy per person - call for fees
Services provided in the home or community (per hour)
Psychosocial Rehabilitation (PSR) - call for fees
Community Psychiatric Support and Treatment - call for fees
What do I need for my first appointment/assessment?
Please bring the following items to the office for you and any minors who are being seen.
ID (Driver’s license or state issued ID)
Social Security Card
Primary Care Physician (Name and Phone)
